How to use
Using your Health Discounts membership benefit is easy. Follow these 3 easy steps and start using your membership benefit today.
Step 1
To see which providers participate in your Health Discounts membership, please use the Provider Search Tool or contact our Customer Service Team toll-free at (844) 279-3195 and let a customer service representative guide you through the process of finding a provider. If your current provider doesn't participate in your Health Discounts membership, you may also nominate that provider to participate using the Provider Search Tool.
Step 2
When booking an appointment with a dental, vision, hearing or prescription provider, please provide your name and membership ID number to the participating provider. The provider will verify your participation using Careington's provider portal. Also, be sure to present your Health Discounts membership ID card at the time of your appointment.
Important note: When using your discount or booking your appointment, be sure to reference the name of the correct network:
- Maximum Care Discount or DenteMax (Dental)
- EyeMed and VSP (Vision Care)
- amplifon Hearing Health Care (Hearing)
- Medimpact Healthcare Systems (Prescriptions)
Step 3
Receive your discounted fee and pay the provider at the time of service. If you have any other questions about how to use your Health Discounts membership, call us toll-free at (844) 279-3195 between 7 a.m. and 7 p.m. CT Monday through Friday for prompt and friendly assistance.
