How to Access the Discounts:
- Locate the EyeMed provider most convenient for the member by calling Member Services at (844) 440-3462 from 7AM-7PM CT or by clicking the Provider Search button.
- Members must identify themselves as an EyeMed member and present their membership card when scheduling an appointment with a participating provider.
- Member must tell the provider their name, the plan number (9247362) and member ID located on their membership card.
